These hidden factors are kept secret by Valve. But let me help you to explain the how is the Dota 2 ranked & Dota 2 ranks are working.
So, let's start with the basics. Dota 2 ranks and a calculation guide.
First of all, MMR is not the only factor determining a rank. Because of this, we can't calculate a rank exactly, and our calculation may change. Also, your MMR will not match to your medal.
There are 8 Dota 2 ranks currently in the game :
Herald, ~0-660 MMR
Guardian, ~770-1430 MMR
Crusader, ~1540-2200 MMR
Archon, ~2310-2970 MMR
Legend, ~3080-3740 MMR
Ancient, ~3850-4510 MMR
Divine, ~4620-5280 MMR
And, we reached the highest rank, which is (...drum roll) Immortal. Players who reach this rank, get an Immortal medal. Immortal medals have a sparkling particle effect in-game. Also inside Immortal there are other "ranks" :

Now, let's check some statistics. Hero win,ban and pick rates.
In this season (Season 3), the hero with the highest win rate is Underlord, while Lone Druid has the lowest win rate and he is the 3rd least picked hero, with a 1.5% pick rate. Now, compare this to Pudge, who has a 42.74% pick rate.
If you want to pick by KDA ratio, your pick should be Spectre. He got a 4.12 KDA, and if you want to avoid losing, then don't pick Huskar (1.95 KDA).
Also keep in mind that choosing a hero with a high win doesn't mean that you will get a guaranteed win. These are just statistics gathered from the players of Dota 2.
So we talked about ranks and statistics. What's missing? Oh! That's it! A guide, how to achieve a higher Dota 2 rank.
Let's start with the basics. In Dota 2, you can queue to 13 game modes :
1. All Pick (you can choose from the whole hero pool, unranked)
2. Turbo Mode (same as All Pick, but it has some differences to shorten the match time)
3. Ranked All Pick. So, in this mode you can queue to get your first rank. It has some differences compared to the All Pick mode : you can ban in this mode. After the banning phase ends, you can't pick the banned heroes.
4. Single Draft (you can choose from 3 heroes)
5. Random Draft (you can choose from a random 50 hero pool)
6. All Random (you get a random hero)
7. Captains Mode (each team's captain bans and picks heroes for your team)
8. Captains Draft (same as Captains Mode, but the captains can choose from a 27 hero pool)
9. Least Played (you can choose from the least played heroes)
10. Limited Heroes (in this mode, you can only get heroes suitable for new players. I would recommend you to play on this mode if you just started playing Dota 2 or you just want to be better at the beginning.)
11. Ability Draft (pick a hero, but play with 4 random abilities that you can choose from an ability pool. This mode is very funny. You should try it if you are looking to have a little fun. :) )
12. 1v1 Solo Mid (you're facing another player on mid lane)
13. All Random Deathmatch (you get a random hero after you die. This mode has a lot of fun too.)
But what if you try to reach your rank that you dreamed of for months, but you can't, because of toxic, inting, AFK players. We all know that feeling. But we also know a solution for that.
